• Housecalls are designed to meet the healthcare needs of homebound individuals or those that find it difficult to leave the home for doctor's appointments. 

• During a HouseCalls visit, you’ll meet with the same types of professionals you’d see in a doctor’s office- nurse practitioner and medical assistant.

• They’ll answer your health-related questions, perform an assessment of your health and physical exam, go over your medications, create a plan of care focused on your healthcare needs, prescribe needed medications, refill medications, give referrals as needed, order labs and discuss results with you. 

• Meet on your schedule, in the convenience of your home.

• Once they arrive, talk about your health concerns at your pace (the visits are focused on you and you alone). Use the results to help coordinate care with your doctor.
